Creatine is having a mainstream moment.

Once treated as a supplement for bodybuilders and power athletes, creatine is now being discussed in conversations about healthy aging, women’s health, cognition, menopause, and long-term physical function. Recent media coverage continues to call it one of the most evidence-backed workout supplements, while new 2025 and 2026 research has expanded the discussion well beyond the weight room.

That creates an interesting problem: when a supplement becomes popular enough, good evidence and exaggerated claims start traveling together.

Creatine deserves much of its reputation. It does not deserve every claim being made about it.

Here’s what the current evidence actually supports.

First: What Is Creatine?

Creatine is a naturally occurring compound made from amino acids. Your body produces it, and you also obtain some from foods, particularly meat and fish.

Most creatine in the body is stored in skeletal muscle as free creatine and phosphocreatine. During short, demanding efforts, phosphocreatine helps rapidly regenerate ATP, the immediate energy currency your muscle cells use to contract.

That is why creatine has traditionally been associated with activities such as lifting, sprinting, jumping, and repeated high-intensity efforts.

Supplementing increases the amount of creatine your tissues can store. And unlike many sports supplements, this mechanism has been studied for decades.

What Creatine Does Best: Help You Get More From Strength Training

The strongest case for creatine remains the least glamorous one: it can improve the results you get from resistance training.

A recent systematic review and dose-response meta-analysis pooled 61 controlled trials examining creatine combined with resistance training. Creatine increased fat-free mass by about 1.39 kilograms on average compared with control conditions.

That does not mean every person who takes creatine instantly gains three pounds of new contractile muscle. Early increases in body mass can include additional intracellular water, and responses vary between individuals.

But over time, creatine can also help you perform a little more high-quality training. An extra repetition here, slightly better repeated-set performance there, and improved training capacity accumulated over months can matter.

This is an important lesson because supplements are often marketed as substitutes for fundamentals.

Creatine isn’t.

It works best as an amplifier of good training, not a replacement for it.

Myth #1: Creatine Makes You Fat

No.

Creatine may increase scale weight, particularly when you first begin taking it. That is not the same as increasing body fat.

Creatine draws water into muscle cells as muscle creatine stores rise. The recent 61-trial meta-analysis found increases in fat-free mass and body mass without significant increases in fat mass or body-fat percentage.

If the scale rises slightly during your first few weeks while your waist measurement is stable, that does not mean your fat-loss plan has failed.

This is one reason judging progress from a single metric can be misleading.

Myth #2: Creatine Is Just for Young Men

This misconception is disappearing quickly, and for good reason.

A 2025 review focused specifically on women’s health concluded that creatine has potential applications across female life stages, with evidence supporting benefits for strength, exercise performance, and body composition, particularly when supplementation is combined with resistance training.

Then, in May 2026, a systematic review and meta-analysis examined randomized trials specifically in postmenopausal women.

Across seven trials involving 608 participants, creatine produced a small but statistically significant increase in lean mass. Leg-press strength also improved, with the pooled analysis showing an approximately 7.5-kilogram greater improvement.

The clearest benefits appeared when at least 5 grams per day was combined with resistance training.

That’s an important qualification.

Taking creatine while remaining sedentary is not equivalent to taking creatine while progressively challenging your muscles.

What About Bone Health?

This is where enthusiasm can get ahead of the evidence.

Creatine is sometimes promoted as though it has been established as a bone-building supplement, particularly for women after menopause.

The 2026 meta-analysis did not find a clear overall improvement in bone mineral density.

That doesn’t mean future research won’t identify specific benefits or populations that respond differently. It means the current evidence doesn’t justify promising that creatine will strengthen your bones.

For now, progressive resistance training, appropriate impact exercise when suitable, adequate nutrition, and medical management of osteoporosis risk remain much more important.

Myth #3: Creatine Is a Proven Brain Booster

This claim needs nuance.

Your brain uses a tremendous amount of energy, and creatine participates in cellular energy metabolism there as well. That makes the hypothesis biologically plausible.

A 2024 systematic review and meta-analysis of 16 randomized controlled trials involving 492 participants found statistically significant improvements in memory, attention time, and processing-speed time.

Sounds impressive.

But the same analysis found no significant improvement in overall cognitive function or executive function. The authors rated the certainty of evidence for memory as moderate, while evidence for several other cognitive outcomes was low certainty.

Another 2024 systematic review was even more cautious, concluding that cognitive benefits appear more plausible under stressed conditions such as sleep deprivation and that evidence in non-stressed individuals remains limited.

So the evidence-based conclusion isn’t:

“Creatine makes you smarter.”

It’s closer to:

Creatine may benefit certain aspects of cognition in some circumstances, but the brain-health evidence is considerably less established than the strength and exercise-performance evidence.

Do You Need a Loading Phase?

No.

A traditional loading protocol uses roughly 20 grams per day, divided into several smaller doses, for about five to seven days before switching to a maintenance dose.

Loading saturates muscle creatine stores faster.

But if you’re not in a hurry, simply taking a smaller daily dose will eventually accomplish the same basic goal.

For most recreational lifters, 3 to 5 grams of creatine monohydrate per day is the simplest approach.

Consistency matters more than elaborate timing.

Do You Need to Take It Before Your Workout?

Probably not.

Creatine doesn’t work like caffeine. You aren’t taking it for an immediate stimulant effect.

The goal is to maintain elevated tissue creatine stores over time.

Morning, afternoon, before training, after training: choose the time you’re most likely to remember.

This is where behavior science becomes more useful than supplement optimization.

If you consistently forget your creatine, attach it to something you already do every day: your morning coffee, breakfast, protein shake, or post-workout meal.

The best supplement schedule is the one that becomes automatic.

Which Type Should You Buy?

Creatine monohydrate.

You can spend more money on exotic forms with impressive names, but creatine monohydrate remains the standard against which other forms are compared and is overwhelmingly the form used in the research literature.

Look for a simple product from a reputable manufacturer. Third-party certification such as NSF Certified for Sport or Informed Sport can provide additional assurance regarding product testing, particularly for competitive athletes.

What About Safety?

Creatine has an extensive safety record in healthy adults when used at standard doses.

The 2026 postmenopausal-women meta-analysis found adverse events were mild and similar to placebo, with no meaningful changes in renal indices.

Still, “safe for healthy adults” should never be translated into “appropriate for every person under every circumstance.”

People with kidney disease, significant medical conditions, pregnancy, breastfeeding, or medications that affect kidney function should discuss supplementation with an appropriate healthcare professional.

Also remember that creatinine, a laboratory marker commonly used when assessing kidney function, is related to creatine metabolism. Supplement use can complicate interpretation of certain lab results, so tell your healthcare provider that you take creatine.

The Lean by Habit Approach to Creatine

The supplement itself is simple. The mistake is allowing it to distract you from the behaviors producing most of your results.

If your goal is to build a leaner, stronger body, prioritize these in order:

  1. Progressive resistance training. Train consistently and gradually increase what your muscles can do.
  2. Adequate protein. Build your diet around sufficient high-quality protein distributed across the day.
  3. Energy balance. Maintain a calorie deficit when losing fat and an appropriate energy intake when prioritizing muscle gain.
  4. Sleep and recovery. Supplements cannot compensate for chronically inadequate recovery.
  5. Daily movement and cardiovascular fitness. Walk, move, and train your cardiovascular system.
  6. Then consider creatine. Three to five grams of creatine monohydrate per day is a practical evidence-based option for many healthy adults.

Notice where creatine appears on that list.

It isn’t first.

But unlike most items in the supplement aisle, it deserves to be on the list at all.

The Bottom Line

Creatine’s current popularity is unusual because the hype is built around a supplement with a genuinely strong scientific foundation.

But that doesn’t make every claim true.

The evidence is strongest for improving high-intensity exercise performance and enhancing adaptations to resistance training. Newer evidence is encouraging for women, including postmenopausal women, particularly when creatine is paired with strength training.

The cognitive research is intriguing, but it’s not yet strong enough to call creatine a proven brain-enhancement supplement. And claims about improving bone density remain uncertain.

That’s not disappointing.

It’s what good fitness advice should look like: use what works, understand its limitations, and never let optimization distract you from consistency.

Creatine may help you get a little more from your training.

Your habits determine whether there’s good training to amplify in the first place.
